Although many students feel overwhelmed applying to college, proper planning, organization and documentation can relieve a lot of stress and make the process much more enjoyable. Senior Survival Kit - College Application Process
This packet will cover all the basics and give you a broad understanding of how to apply to college, the information you will need, what your responsibilities are and what your guidance counselor will do. It should be a useful guide and a reminder to get everything done in a timely manner.
Parent Waiver - Transcript Release Form
Before a transcript can be released by East Stroudsburg High School South to any post-secondary school, this form must be filled out and signed by a parent or guardian. Before beginning the college application process, seniors should have this form on file in the guidance office. Even if a student is 18 years old, a parent or guardian's signature is required.
Transcript Tracking Form (Google Form)
This form is used to request that transcripts and /or supplemental documents be sent to any post-secondary institution, branch of the armed services, NCAA clearinghouse, scholarship provider, potential employer, etc. A new tracking form must be filled out for each request and must be signed by a parent if the student is under the age of 18. Transcript requests are free for students currently attending East Stroudsburg School District.

Non-School Sponsored Educational Trip Permission Form for College Visits
If you are planning a college visit, please fill out the Non-School District Sponsored Educational Trip form in order for it to be an excused absence. This is only good for a total of three school days whether it is for one trip or three separate trips.
